EMDR Research News May 2014

This month I bring you five new articles directly related to EMDR therapy and two bonus articles addressing the neurobiology of PTSD.

Among the five EMDR therapy articles, there are two
case reports. These examine EMDR therapy for mild dementia and for complex regional pain syndrome. There is a research report from the Netherlands showing that the eye movements used in EMDR therapy produce enduring changes in memory but only when there is sufficient duration of the eye movements. There is a Clinical Q & A article on a new metaphor for helping clients understand their issues and recognize their gains in the EMDR approach to therapy. Finally, there is a clinical overview of how to apply EMDR therapy to cases of borderline personality disorder from my Spanish co-authors, Dolores Mosquera and Anabel Gonzalez and myself.

In addition you will find a summary of a French article on magneto-encephalographic (MEG) brain recordings during traumatic memory recall in women with post-traumatic stress disorder which is dedicated to the memory of our colleague David Servan-Schreiber who died in 2011. Finally, there is full text link to an in-depth article on a theoretical model of the neurobiology of the hyperarousal subtype of PTSD.

EMDR Research News April 2014

Due to a busy travel schedule there was no time for a March research listing this year. As of early April there are seven new journal articles since our February update on EMDR therapy, two recently released books for EMDR trained clinicians, and a wonderful video about a simple (and research supported) way to increase personal presence.

Among the seven articles, there are three
case reports. These examine EMDR therapy for trauma with mild intellectual disabilities, for treating complex trauma, and for adult survivors of sexual abuse.

There are two
research articles: the first a potentially highly significant randomized controlled trial with combat veterans from the Iraq war comparing CBT and EMDR; the other a follow-up report on outreach efforts after the 2005 bombings in London that included some EMDR treatment.

There are two
review articles: the first is a major review article by Francine Shapiro in The Permanente Journal addressing applications of EMDR in medicine; the other reviews treatment approaches for PTSD in Germany.

The
video of the month features an inspirational Ted Talk with social psychologist Amy Cuddy on a simple way to increase personal presence.

In the

EMDR Research News November 2013

This month there are five new journal articles and two commentaries. First, there is a quasi-experimental study of EMDR therapy for students with speech anxiety. There is a randomized controlled comparison of EMDR and progressive counting for single-incident trauma or loss. There is a review article that mentions EMDR in relationship to perceived treatment helpfulness in cohort of World Trade Center rescue and recovery workers. Then, there is a curious randomized controlled treatment outcome study from Iran comparing EMDR therapy with prolonged exposure therapy for combat veterans suffering from chronic PTSD. It is curious because subjects who received PE were given twice as many treatment sessions as those receiving EMDR. And, there is another experimental study from the working memory group in the Netherlands on the blurring of emotional memories through eye movements.

Finally there is a commentary critical of the 2013 Lee and Cuijpers meta-analysis of the specific effects of eye movements in both laboratory and EMDR therapy outcome studies along with a rebuttal from Lee and Cuijpers. A preview of the first page of each of these commentaries is open access. These commentaries stand clearly on their own merits, but merely as an introduction I would offer you this quotation from Arthur C. Clark:

"It is really quite amazing by what margins competent but conservative scientists and engineers can miss the mark, when they start with the preconceived idea that what they are investigating is impossible. When this happens, the most well-informed men become blinded by their prejudices and are unable to see what lies directly ahead of them."

This month I am bringing you the introduction to a video recording of Debra Wesselmann’s 2010 workshop at the EMDR Europe Conference in Hamburg on “
Adapting EMDR for Children with Reactive Attachment Disorder Behaviors.”

In the
forthcoming books section you will find an early announcement of two exciting books from Debra Wesselmann and her co-authors Cathy Schweitzer and Stefanie Armstrong on a team treatment model for offering EMDR therapy to parents and their children with attachment and trauma issues.

EMDR Research News October 2013

This month there are eight new journal articles, six on EMDR therapy and two studies of eye movements. There are two randomized controlled trials of EMDR therapy from Europe: the first is on EMDR vs. wait list for dental phobia and the second is on EMDR vs. prolonged exposure for PTSD in patients with psychosis. Also from Europe, there is a single case study of a sex offender and two general review articles. Then there are two non-clinical, laboratory studies of eye movements, both claiming to be studies of EMDR: the first with negative findings on the working memory hypothesis and the second examining defenses against negative messages about eating fruits and vegetables. (No joking, it really is a study about coping with negative messages about eating fruits and vegetables.) Finally there is a psychoanalytic critique of EMDR and its theory.

EMDR Research News September 2013

I am especially pleased to bring you this month’s recently published, peer-reviewed journal articles related to EMDR.

This month there are five articles from the special issue on
Breaking New Ground of Volume 7 Issue 3 of the Journal of EMDR Practice and Research (JEMDR) for which I served as guest editor. While I do not normally include listings for editorials, I make an exception to also list my editorial below. Members of EMDRIA can access my editorial on “The Future of EMDR” in the Journal of EMDR Practice and Research in the member’s area on the EMDRIA website.

This special issue includes two controlled pilot studies of EMDR versus CBT. The first by Faretta examines the treatment of panic attacks. This is the first direct treatment comparison of panic by EMDR and CBT. The second controlled study by Capezzani, et al. explores the application of EMDR for cancer patients. Amano et al. present a case report of a woman with phantom limb pain after anesthesia failed during surgery in which near-infrared spectroscopy was used to identify real-time brain changes during successful EMDR treatment. Gauvry et al. present a case report of successful hospital based treatment of complex regional pain syndrome (CRPS) in a boy. Finally, there is a case study from Italian psychologists Maria Zaccagnino and Martina Cussino of a depressed mother who presented with a dismissing attachment style (with regard to both parents) who showed adaptive changes in caregiving capabilities and attachment organization following treatment with EMDR as measured by the Parent Development Interview.

There is also an abstract for a registered research proposal to study EMDR with non-specific chronic back pain in a randomized controlled trial. EMDR Research News August 2013

Here are this month’s recently published, peer-reviewed journal articles related to EMDR. This month there are three new articles. In the first of these, de Jong et al. (2013) compare the effectiveness of EMDR with bilateral eye movements or tones with recall only (watching a blank wall) in a “real world” study from the clinical practices of Dutch EMDR therapists. Interestingly they found equivalent benefits for those with either PTSD or other diagnoses. Clinicians and clients who prefer bilateral tones to eye movements, may find the results of this "real world" study challenge their assumptions. Read the abstract below for the provocative results. Then there are two review articles. In the first, Gillies et al. (2013) publish their review of psychological therapies for PTSD in children and adolescents. In the second, Verstrael et al. (2013) report their meta-analysis of (EMDR) as treatment for combat-related PTSD. In both cases, the findings make it clear that there needs to be much more research with EMDR.

On a brighter note, the
World Health Organization released new clinical protocols and guidelines to health-care workers which recognize both EMDR and trauma focused CBT for treating the mental health consequences of trauma and loss in children, adolescents, and adults. Below the break you will find how to get free PDF copies and a short commentary on their findings.

EMDR Research News July 2013

Here are this month’s recently published, peer-reviewed journal articles related to EMDR. Following June’s first month in memory with no new research articles -- and no blog entry -- this month there are four new articles.

First, there is an exciting controlled comparison investigating early, immediate (within 48 hours) EMDR treatment for victims of workplace violence compared with eclectic therapy. Second, there is a registry description of a planned Dutch study to examine random assignment to prolonged exposure, EMDR and waiting list for patients with a current diagnosis of psychosis and co morbid post traumatic stress disorder. Third, there is a preliminary set of two cases in which two veterans with chronic PTSD, received four weekly sessions of 3MDR therapy - combining virtual reality exposure with EMDR while on a treadmill. Finally there is a meta-analysis in
The Journal of Clinical Psychiatry reporting that effective psychotherapies with similar treatment effect sizes included cognitive therapy, exposure therapy, and EMDR and that effective pharmacotherapies, with much smaller treatment effect sizes, included paroxetine, sertraline, fluoxetine, risperidone, topiramate, and venlafaxine.

EMDR Research News January 2013

Here are this month’s recently published, peer-reviewed journal articles related to EMDR. This month there are 17 new articles of interest to EMDR trained clinicians and researchers.

There are seven new
treatment reviews mentioning EMDR. Of these, three are new reviews of trauma-informed treatment for adolescents (Black et al., 2012; Gillies et al., 2012; Greyber et al., 2012); plus, there are reviews of treatments for psychogenic nonepileptic seizures (Baslet, 2012), of women veterans with insomnia and PTSD (Hughes, et al., 2012), and sleep disturbances in PTSD (Schoenfeld, et al., 2012). There are two general reviews of EMDR: EMDR for post-traumatic stress (Norgate, 2012) and EMDR as a neurorehabilitation method (Zarghi, et al., 2013). There are two EMDR single case reports: sexual trauma and contamination fears (Nijdam et al., 2012), and treating violent impulses in a military client (Wright and Russell, 2012). There are two EMDR case series reports. The first is a series involving brief assessment and treatment from the Democratic Republic of the Congo of sexual trauma and mutilation (Mankuta et al., 2012) and from South Korean there is a series examining plasma levels of brain-derived neurotrophic factor and nerve growth factor before and after eight sessions of EMDR (Park, et al., 2012).

Then there are a series of five “non-EMDR articles” of potential interest to EMDR clinicians and researchers. First there is an article and a commentary on legal and ethical issues related to acute disaster mental health service which mentions EMDR (Call, et al., 2012; Howe, 2012). Then there are two non-EMDR randomized controlled trials. The first is of a manualized hypnotic treatment for
Ego State Therapy (Christensen, et al., 2012). The second examines treatment effects on insular and anterior cingulate cortex activation during classic and emotional Stroop interference in child abuse-related complex post-traumatic stress disorder (Thomaes, et al., 2012) -- a research paradigm that has been used in some EMDR studies. Finally I include a listing for one non-EMDR review of exposure based treatment for PTSD (Rauch et al., 2012).

This month I feature the video (audio only) of

EMDR Research News December 2012

Here are this month’s recently published, peer-reviewed journal articles related to EMDR. There are 9 new EMDR articles, including one full text, open access article on EMDR treatment of addiction. I am also pleased to announce the publication of the Spanish edition of my 2009 book A Guide to the Standard EMDR Protocols for Clinicians, Supervisors, and Consultants.

Several other articles of interest appear this month. Perhaps most crucial to EMDR's scientific acceptance is the long awaited meta-analysis from Chris Lee and Pim Cuijpers examining 15 clinical and 11 laboratory trials in which there was both an EMDR treatment and a no eye movement condition. They found "the additive effect of eye movements in EMDR treatment studies was moderate and significant." While this may seem like old news to EMDR clinicians who depend on this effect for clinical purposes, funding for EMDR research in the USA has languished in the long shadow of Davidson and Parker's (2001) statistically flawed paper which claimed eye movements made no difference over "exposure". Lee and Pim (2012) conclude, "Results ... suggest the processes involved in EMDR are different from other exposure based therapies."

Those who were concerned about Hornsveld et al.'s 2011 paper "Evaluating the Effect of Eye Movements on Positive Memories Such as Those Used in Resource Development and Installation" in Volume 5 number 4 of JEMDR, will want to read the commentary by Leeds and Korn "A Commentary on Hornsveld et al. (2011): A Valid Test of Resource Development and Installation? Absolutely Not" and the rebuttal by Hornsveld, de Jongh and ten Broeke "Stop the Use of Eye Movements in Resource Development and Installation, Until Their Additional Value Has Been Proven: A Rejoinder to Leeds and Korn (2012)" in Volume 6 number 4 of JEMDR. See the abstracts below.

EMDR Research News July 2012

Here are this month’s recently published, peer-reviewed journal articles related to EMDR. This month we find all European reports: two surveys on the use of specialized methods of treating trauma, one from from Germany (Kliem et al, 2012 for Type II trauma) and one from the UK (Murphy et al., 2012 for NICE informed specialty trauma services); three case reports from Switzerland and France on EMDR treatment of illegal drug use, victims of domestic abuse, and domestic partner rape; a laboratory study from the working memory group in the Netherlands of the effects of eye movements on reaction times for “future recall”; and two theoretical papers. Rachel Worthington (2012) proposes that violence is often the result of traumatic experiences and that EMDR should be considered preferable to exposure and TF-CBT when treating traumatic origins of violence. Finally Hans Welling proposes that methods such as EMDR, based on reconsolidation are preferable for “transforming maladaptive emotions, whereas exposure is most appropriate for accessing disowned and avoided experiences.” After reading Welling’s abstract below, readers may want to review my February 2010 essay on “Extinction or Reconsolidation: Differences between suppression and transformation in how we recover from traumatic experiences.

Developmental Pathways to Dissociation

Is early trauma the root cause of dissociative disorders?
Most of the literature and research on dissociation focuses on trauma as the root cause of dissociative disorders, and many studies show an association between dissociation and trauma (Putnam, 1997). “The fact that nontraumatized individuals sometimes demonstrate dissociation and that not all trauma survivors dissociate suggests that there may be more to the etiology and development of dissociation than trauma alone.” (Dutra, Bianchi, Siegel and Lyons-Ruth, 2009, p. 84). If early trauma is not the unique cause of dissociation, then what else could cause it?

Extinction or Reconsolidation
Differences between suppression and transformation in how we recover from traumatic experiences

Memory is central to all learned behavior, and in humans to one’s sense of identity. Sleep and memory processes are deeply entwined. Both are central to our sense of well-being. As Shakespeare wrote in Macbeth,

Sleep that knits up the ravell'd sleeve of care,
The death of each day's life, sore labour's bath,
Balm of hurt minds, great nature's second course,
Chief nourisher in life's feast

Problems with memory processes (and with sleep) are at the root of one of the most pervasive problems in society, posttraumatic stress disorder (PTSD). Those with PTSD suffer from memories they cannot escape nor resolve. They cannot escape into sleep because their memories follow them into recurrent nightmares.
